Ream and run - hunting and handball

A 56 year old man presented with shoulder pain. His shoulder arthritis is shown on the x-rays below.





Typically, his axillary 'truth' view, shows a biconcave glenoid with posterior decentering.




Even though he could perform 8 of the 12 SST functions, he elected a ream and run procedure because of difficulty playing his favorite sport (handball).

Here are his post operative  x-rays showing a radiographic joint space between the prosthetic humeral head and the reamed glenoid.

 And centering of the humeral head on the axillary 'truth' view.


Yesterday, we received this email progress report from him:

"Hope all is well. Update at 21 weeks:
Flexibility remains good. Stretches are much easer. Daily stretching frequency 1-2 sessions per day to maintain.
Continue to gain strength. Back or pulling exercises are almost back to pre-surgery limits. Being more conservative on bench type moves as I’m still cautious about protecting the subs cap. Up to 20 lb. dumb bells on the incline bench at 20 rep sets. Could do more but taking it slowly. Starting military dumb bell press as well.
Swimming continues to be a mainstay in weekly routine. 500 - 1,000 meters freestyle sessions depending on other activities that day.
Throwing handball against the wall very comfortably for last 6 weeks. Starting to add smooth back wall shots and soft front court shots. Progressing conservatively as not yet ready for aggressive movements. Smooth movements are key.
No pain meds. Occasional Aleve if I sense stiffness following long workout.

First real test in two weeks with the side-by-side. Headed to West Texas for a dove shoot with friends to kick off the fall bird season. Will stop shooting the 28 ga. if I feel any significant discomfort. Any risk from your perspective?

Really am pleased with this entire process. Hard to believe we are approaching 5 months. Have my first handball match scheduled for March 28, 2018 which is one year surgery anniversary. Based on where I am at now, hoping we can move that up to the end-of-the-year. Lost my last match before surgery so I can’t finish the year on that note. Will keep you posted."
