Is preoperative planning of reverse total shoulder helpful?

Assessment of surgeon variability in preoperative planning of reverse total shoulder arthroplasty: a quantitative comparison of 49 cases planned by 9 surgeons

These authors measured inter- and intrasurgeon variability in preoperative planning using Exactech GPS for 49 reverse total shoulder arthroplasties (RTSA ) to identify differences in how 9 fellowship trained surgeons consider optimal implant placement. the same platform

The results were inconsistent among the different surgeons.



Interclass correlation coefficients for intersurgeon variability ranged from 0.43 for version, 0.42 for inclination, and 0.25 for baseplate type. Pearson coefficients for intrasurgeon variability were 0.34 for version and 0.30 for inclination. Light’s kappa coefficient for baseplate type was 0.61. 
When the same surgeons repeated their planning, the results were inconsistent.
























The authors concluded that surgeons differ considerably in what they consider to be an optimal  reconstruction and that an individual surgeon’s ideal reconstruction for a given case differs with repeated attempts at planning.
Their analysis not only showed differences in frequency and threshold for implant use but also differences in the degree of correction and range of residual retroversion and inclination after correction.Variability did not appear to be a function of the severity of glenoid deformity and existed across the spectrum of morphology.
Comment: It would be of interest to know how much time the surgeons spent in their planning as well as the cost of the planning software and its implementation.

This study indicates that the anatomic objectives of preoperative planning technology, the indications for its use, and its value to the patient remain undefined.
