Ream and run for post traumatic arthritis

A patient from southern California presented for treatment of a decade-old shoulder deformity from a fracture and prior surgery. Note on the x-rays below that the humeral head had completely collapsed and the glenoid articular surface had become convex.

 The proximal humerus was posteriorly displaced on the humeral shaft.
Because the desire to resume vigorous physical activities, the patient selected a ream and run procedure.
We used a posteriorly eccentric head to achieve normal glenohumeral relationships.
