Severe B2 glenoid in a young man

A 40 year-old man presents with a painful stiff shoulder. He had had a prior surgery to remove a loose body. His x-rays show a large loose body, osteophytes, and flattening of his humeral head.

 His axillary "truth" view shows posterior decdentering of the humeral head on a biconcave glenoid.

He elected a ream and run procedure. No preoperative CT was performed. Multiple loose bodies were removed


 An anteriorly eccentric humeral head was necessary to manage excessive posterior translation at the time of surgery.

One week after surgery he send this photo nicely showing his range of motion.
